This document is approximately five pages in length.  All pages are narrow - 8.5" X 11" in rich text format.

A file is provided for you to download.  It is formatted in rich text and compressed using a self-extracting utility by Winzip.  After downloading the file, just open the file and it will self-extract to the folder or directory of your choice.  The default is \windows\temp.
